Scheduling Sessions
Sessions are typically held 2-4 times per month, although the frequency may vary depending upon your needs.  Our meetings are held by appointment and they are 45 minutes in length unless otherwise specified.  That period is reserved for you, so I ask that you please provide at least 24 hours advance notice if you must cancel.  If such notice is not given you will be billed for the session.  If you miss multiple sessions we may need to discontinue our meetings.

I maintain notes regarding our sessions in a secure location.  The matters we discuss will remain confidential.  I will not share information about your treatment without your prior consent unless I am legally compelled to do so.  For example, I am bound by law to report instances of suspected child/elder abuse, and I may need to notify others if I believe you are in danger of harming yourself or someone else.  Illinois law also requires that I report firearm holders who pose a clear and present danger to themselves or others.

There are certain other legal situations in which I may need to share information about you (e.g. if ordered to do so by a court), and I urge you to inquire about this if you have any concerns.  I may find it helpful to consult with another health professional about your case, but I will make every effort to avoid revealing your identity and the consultant will be legally bound to keep the information confidential.

In order to ensure your privacy and to prevent insurance companies from intruding upon our work together, I no longer accept insurance reimbursement and payment is requested at the end of each session.

I will provide you with a receipt so that you may file an insurance claim with your carrier if you wish. Effective 3/1/19 the standard fee is $250 for the initial session and $195 for subsequent 45-min sessions, payable at the time of service.  The latter fee also applies to other professional services such as report writing, telephone conversations lasting longer than 5 minutes, consulting with other professionals at your request, or preparation of records.
